“Penis pumps are mechanical devices that are used to achieve and maintain an erection by creating a vacuum,” explains sexologist and GP at Doctify, Dr Alessio Platania. “They are usually shaped like a cylinder to be put over the penis. Removing the air pulls blood into the penis, resulting in an erection.”

This can also result in increased sensitivity and temporary penis enlargement.

“Once this is done, a constriction ring can be placed at the base of the shaft to maintain the erection while engaging in sexual activity, if needed,” says Platania.

The benefits of using a penis pump

There are a few reasons why people use penis pumps, but they’re primarily used to increase the length and quality of an erection. “A penis pump uses negative pressure to suction blood into the blood vessels around your penis,” explains Zachary Zane, Cosmopolitan US columnist and author of Boyslut.

This can result in a firm erection, which is particularly helpful for those who struggle to maintain an erection during sex.”

This makes penis pumps the go-to choice for those experiencing erectile dysfunction (ED) who want a cost-effective, non-invasive treatment, or who are looking to regain function after a medical procedure.

“Using a penis pump helps improve the blood circulation of the penis' tissues, improving oxygenation and penile health,” says Platania. “After surgery, it helps prevent shrinkage of the penis and restore function. Other benefits are ease of use, being a non-invasive method, that they can be used when other ED medications aren't recommended, and — despite the initial cost — one pump can last several years.”

“The next thing you want to look for is the size, as you need a good grip at the base to create the air seal, but enough space to fit your fully erect penis without damage.” So measure up!

How to use a penis pump

1. Lubricant

You can take or leave this first piece of advice, but some experts suggest shaving your public hair a little before using a penis pump to make the seal between your skin and the device more effective. It also helps to stop hair from getting caught.

Once you’ve done that – or not, as the case may be – then you can apply a thin layer of water-based lubricant over the base of the penis to prevent irritation. If you didn’t shave before, you’re in luck: the lube actually helps to sweep your pubic hair back and out of the way.

“Apply water-based lube at the base of the penis to help create the seal, put the pump on top of the penis, and then gradually operate it to create the vacuum and obtain the erection,” says Platania.

2. Pump!

There are many different ways to operate pumps. Hand-pumps are fairly self-explanatory: you squeeze the pump to create the vacuum. Trigger-action pumps work in the same way, only they come with a little pull-trigger to help you along. And, finally, battery-powered pumps do it all for you.

No matter which kind you’re using, they all work in essentially the same way – you’ll just need to squeeze, pull, or press the button to get the device (and your penis) pumping.

Once you’re in, don’t be afraid to readjust until you’re comfortable. Plus, don't forget to hold your balls while using the pump, Platania cautions. “If you don't, either or both testicles can be sucked into the pump, which is not a nice feeling,” he says.

“Lastly, it could be more efficient to use the pump just before your shower to clean all the lube and the equipment.”

3. Take it slow

Although it may be novel using a penis pump, particularly the first time, don’t get overexcited. For the best and most comfy results, build up the pressure slowly while feeling it out.

Most people tend to get an erection between 30 seconds and three minutes, depending on their bodies, as well as the device they’re using. Once you’re erect, you can stop (more on that in a minute).

Importantly, you should listen to your body; if anything begins to hurt or feel uncomfortable, stop immediately. You should also make sure you're not using the pump for more than the recommended. “Remember, it should NOT be kept on for more than 20-30 minutes,” Platania explains. If in doubt, follow the instructions on your pump’s manual.

4. Release

Congratulations, you’ve got an erection! Now you’re ready to stop pumping and, er, start humping.

To finish, hold the base of your penis while pressing the penis pump’s release valve. This will stop suction and enable you to remove the tube.

“If you want to engage in sexual activity afterward, you can slide a constriction ring [AKA cock ring or stamina ring] at the base of the penis before removing the pump,” says Platania.

And don't be alarmed if things feel a little different down there than usual.

“It is good to be aware of the fact that erections obtained this way will feel different and colder than regular erections, but are good enough for penetration,” Platania says. “If used for sexual intercourse, ejaculation might be blocked by the constriction ring; this is safe, and semen will just go into the bladder and be expelled the next time you pee.”

Do penis pumps actually work?

The results really depend on what you’re expecting, so you have to be realistic. If you’re looking for help getting erections — then yes, they work, and fast. If you’re looking to increase sensitivity and enjoy the visual of it growing, then you’re definitely going to be satisfied.

“Penis pumps are used with very high success and satisfaction for erectile dysfunction issues, where medical therapy with pills like Sildenafil (Viagra) or Tadalafil (Cialis) is not recommended, not working, and the patient prefers to avoid more invasive methods like penile injections or surgery,” Platania explains.

They can also be great following surgery to the penis or prostate, or used to treat Peyronie's disease, Platania adds.

However, a penis pump is not going to give you a bigger erection, contrary to popular belief. There are varied reviews out there but there’s no evidence to back up these claims. Sex toy influencer Dick Baker claims, “I noticed once I’d been pumping for a month or so that flaccid my penis had increased in length and girth by a small amount.” But if this was widely true, we think most of the penis-owning population would be permanently connected to one.

Are penis pumps safe to use?

Although they’re safe to use, like with anything, there are some potential risks with using penis pumps. “This includes blood clots and scarring,” explains Zane, “but this is rare and only tends to happen with improper use.”

He points to a 1989 study which found that, out of 1,500 penis pump users, not a single person reported any serious adverse effects.

When used correctly, the risk of side effects is low. “Penis pumps are very safe when used according to instructions; however, as with all medical devices, they are not suitable for everyone, and you should always discuss it with your clinician,” Platania says.

“As the mechanism of action interacts with the blood flow in the genitals, people who have some blood disorders or are taking medications that can affect blood thickness might not be eligible. Apart from this, side effects are usually mild and temporary, like swelling or the appearance of small bruises or red spots due to burst vessels with the pressure.”
